If your group is not fully formed, that is okay. Submit the names you have now and come back to this form any time you have a new group member or members to add. Please add your address and first estimated gathering date the first time you use this form. Then, as you add more names, please type N/A in for the address and first gathering date sections.

Each time you submit this form, the name(s) you’ve provided will receive a Welcome Email with their login details to The Invitation portal.
